BANK DRAFT AUTHORIZATION
I authorize Business Information Systems (BIS) or Tennessee Department of Revenue to withdraw (debit) funds from the bank account provided in this form on a monthly basis for use of the EZTAG service, which are to be remitted by BIS or directly taken by the State of Tennessee on my behalf to the State of Tennessee Department of Revenue. I agree to use the reporting features within the EZTAG online interface to reconcile the monthly withdrawals.
TERMS AND CONDITIONS
Monthly debits will be based on the entries (temp tags printed) that have been posted in the previous month. Monthly debits to your bank account will be made via ACH transactions. ACH transactions may fail to process for a number of reasons, including but not limited to insufficient funds and invalid account information. You will be notified via email if there is an ACH failure (may be called “ACH return”). You must then use the EZTAG web interface to correct bank account information if necessary, and then post the associated entries again within one business day after receiving notice of the ACH failure. BIS and the State of Tennessee reserve the right to terminate your access to the EZTAG service at any time. Termination of access to the EZTAG service will not absolve you from liability to the State of Tennessee Department of Revenue for any unpaid entries. BIS does not guarantee payment to the State of Tennessee Department of Revenue on your behalf. If BIS should fail to obtain the funds from your account for any reason, you may be required to remit payment to the State of Tennessee Department of Revenue by other means.
LIMITATION OF LIABILITIES
Under no circumstances will BIS be liable to any party for any direct, indirect, special, incidental, consequential, or other damages based on any use of this system or any other web site to which this site is linked.
PRIVACY
Privacy on the web is a priority. We inform our visitors about the types of information we collect and how we use it. We save messages we think might be needed for customer call back. We only keep email addresses on file for our customer service records. We DON'T sell or give out email addresses to companies.
REFUNDS
Any refunds must be approved by the State of Tennessee Department of Revenue, and may be processed by BIS or the State of Tennessee directly.
STATE OF TENNESSEE TERMS AND CONDITIONS
EZTag ACH Terms and Conditions
As a duly authorized check signer on the financial institution account identified below, I authorize Tennessee
Department of Revenue (“DOR”) to perform scheduled or periodic electronic funds transfer debits and/or credits from my
account identified below for payment due the State of Tennessee for monthly EZTag accounts.
The sum to be transferred will be debited from the account specified below based on calculations of amount due sold
thru the EZTag application. The calculations are based on the state authorized fees for products and services in
effect at the time of the transaction. The exact amount of the fee amount to be transferred is available to me through
reports in the EZTag system and I hereby acknowledge the availability of the reports.
I understand that to properly remit the amount specified to the Tennessee Department of Revenue in the process above
I will insure sufficiency of funds in the account. I also understand that I am responsible for updating the Financial
Institution information below immediately on effect of a change.
DOR RESERVES THE RIGHT TO TERMINATE YOUR ACCESS TO THE EZTAG SERVICE AT ANY TIME. TERMINATION OF ACCESS TO THE EZTAG SERVICE WILL NOT ABSOLVE YOU FROM LIABILITY TO DOR FOR ANY UNPAID ENTRIES.

EZTag Requirements
Dealers will maintain an active dealer’s license with the Motor Vehicle Commission.
Dealers will use a laser printer and approved dealer tag paper. The approved paper can be purchased from
Business Information Systems.
Dealers will timely submit all required fees to the state using an approved Automated Clearing House (ACH)
payment process.
Dealers will place the printed drive out tag on the back of the vehicle in the same position as a
state-issue license plate (this tag may not be placed in the back window).
This authorization shall remain in effect until I notify Tennessee Department of Revenue in writing to cancel it.
I understand that it may take up to thirty (30) days for Tennessee Department of Revenue and the financial institution
to fully process a request for cancellation.
